How do I apply for a permanent residence visa

No such thing.

You gain 'permanent residence status' in Canada by applying through one of the 150-odd immigration streams and then by continuing to satisfy the ongoing 'residency obligation' tests (basically must then remain in Canada for 2 years in any rolling 5 year period). Which immigration stream do you want to apply through?
