PCC from Qatar Embassy in UAE

Hi
would any of you guys know the procedure for applying for PCC for Qatar,
i previously lived in Qatar for 4 years and know that i can get a PCC through the Qatar Embassy in UAE, where i presently live in...., but what is this "finger print form" any guesses???
and any idea how long would this take?

anyone got PCC for qatar?
can share thier experience please, especially this finger print procedure?

will they ask for : 'where i lived in qatar'? address etc.?

as i dont have any remembrance about the address, just stayed in Plant Camp...
