Passport request email - udpdated information chart not attached

Hello,

I received my passeport request letter by email and the "updated information chart" that I am supposed to complete et return was not attached to the email. I sent CIC an email and also raised a CSE on their website but no answer. Anyone know what this form is and what is the reference ? can I find it on CIC's website? I

Appendix B is the Updated Information Chart. It doesn't come as a separate attachment. It will be part of the Ready for Visa email.
