Passport Number change after Spouse Medical

I have a question, Please help.

I am in the process of sponsoring my spouse this month. I haven't sent the file yet.

My wife did her up front medical in Feb, and her passport was expiring next month, so we applied for reissuing the new passport. Now her new passport has
new passport number with changed surname, But upfront medical report shows her old passport information.

Please Suggest what to do?

Should we do her medical again
or
Attach old passport attested photocopies with the file.

Did you already send in your application and then update her passport? If so, then you can simply send a copy of the new passport to CIC once you have your file number (which you get after sponsor approval).

If you have not sent it in, then you probably can just have an explanation with a copy of her old passport. Hopefully someone else can provide more input.
