I had been to Dubai for couple of months (vacation/visiting frnds) in 2008 and my passport has entry and exit stamps.

Do I need to include a photo copy of that in my package along with my passport first page and last page or not required?

I am also assuming that I don't need PCC from Dubai as it was only 2 months (55 days to be exact) - Correct?

Each checklist specifies if it needs just the "cover" or "cover and all pages with stamps". Read it and stick to what it says.

That is a tricky question. If you were just on a very long tourist stay then no... but that depends on your immigration status in UAE. I would suggest to wait until CIC asks for them, if they ask for them at all.

yes I was there on a tourist/visit visa....

The magic number is 6 months.
Have you lived in a country for more than 6 months - provide a PCC
less than 6 months - no PCC required.
