Hello everyone!

I am currently sponsoring my spouse who right now lives in Massachusetts, USA. I have already sent out the application forms and other documents needed 2 days ago. However, I held off with the part 3 forms which includes the appendix A and B. Can I send these forms now even tho i haven't been issued with the UCI #? I'm worried they may not include it with the other forms I already submitted.

The reason why I didnt send these forms right away cos my spouse have not done the medicals. According the receptionist in the medical office he needs to bring the appendices and they will send it out for him. But I just discovered that panel physicians have now new emedical forms. Also, it doesnt make sense to me why medical office would need a document checklist and photo specifications (App A and B). Im confused and worried. Please someone help.
 
Hi, you should include the checklist in the application as well as the photos. panel physicians don't need the checklist. I don't know if you did that. They just need the meds form and two photos. Meds should be done before you send out the application and copy 2 of it should be included in the application too. Dont't send the meds until you have your file no., otherwise they don't know to which application should they attach this added document.
 
You could try sending them, but there is a risk they may get lost. You can wait until they return your application for being incomplete, but it will slow the process down. Thankfully the sponsorship part seems to be fast, and I think they check for completeness of the file in that step.

Btw how many photos do they require for the medical?

it depends on the doctor apparently, so call ahead to get an appointment and ask what they require ( some ask for specific pictures, others for normal pictures etc).
