Parents have only 1 ID for Passport form

Hello ppl,

I am filling out my parents passport form and notice that under Section 5, I am to provide 2 Documents to Support Identity.

But both of my parents only have their Quebec Health cards. They do not have a drivers license, nor any other type of government issued document.

I wanted to use the PR card but it is taken away after the citizenship ceremony.

What should my course of action be here ?

HealthCard is fine.
Also, bank credit card in some cases can be considered ID, its got name on it, no?

What about their passports from country of origin ??

The instruction said to have at least one ID showing photo, name, date of birth, sex and signature. Two or more are only needed if combining them will satisfy the above requirements. There are two boxes if two ID's must be used to fulfill the requirements.
Country of birth passport will be sufficient if the Quebec health card does not include all the required information.

Just one document is required . Please use the latest version of the forms. On the bottom side of the form you will find the date and according to me 06/2018 is the latest one. If you pick some older ones you could figure out the difference.
