OWP question about forms

I was reading the guide and here in the forums, and just wanna get everything clear.
They give you a list of forms for the OWP which are

• Application to Change Conditions or Extend Your Stay in Canada [IMM 5710] (PDF, 484.21 KB)
• Document Checklist [IMM 5556] (PDF, 767.09 KB)
• Statutory Declaration of Common-law Union [IMM 5409] (PDF, 636.51 KB), if applicable
  • Must be notarized to be considered valid.
  • If you are not in the same country as your common-law partner at the time of signature, each person can present their own declaration and submit the IMM 5409 with the application.
• Use of a Representative [IMM 5476] (PDF, 648.31 KB), if applicable
• Authority to release personal information to a designated individual [IMM 5475] (PDF, 593.57 KB), if applicable

I'm being sponsored by my husband, and we are not using a representative, so that means I only have to send the main form IMM5710 and the Checklist IMM5566?

Yes we sent those two forms plus the statuatory declaration of common law (because we are not married), along with all the supporting documents they mention (passport copy, proof of status in Canada etc)
