option 3 in IMM5710

Hi,

I am on spouse open work permit in Canada since Feb 2020. My status gonna expire BY end of July 2020. My spouse (on study permit) also going to extend his study permit. i am completing IMM5710e form to extend my work permit with the study permit of my spouse. in the form, option3:

"I am applying for one or more of the following":
1. A work permit with the same employer
2. An initial work permit or a work permit with a new employer
3. Restoration of your status as a worker
4. Temporary resident permit

My questions are:
1) what would be my option from above these?
2) since Feb 2020, I could not work in Canada due to covid19. what should i write in the EMPLOYMENT/OCCUPATION section for the current job description? should I mention my back home experience in previous employment or not?

2. You should fill in your last 3 activities employed, unemployed, studying - whatever you were doing, without gaps.

I have one more question. When I give answer to questions on my CIC account before submitting documents. There is one queston:

Are you accompanying a family member who has status in Canada??

what should I answer to this question? as File is submitting through my spouse CIC account.

I've never completed those questions, but if your application is dependent on your spouse's then it seems like the answer would be 'yes'.
