Open Work Permit

Hi Everyone - I am presently working in Canada on a work permit while I await my PR. I am looking into extending my stay as a worker under an open work permit. My question is on the application form, which do I select?

An extension of a work permit with the same employer
A work permit with a new employer
Restoration of your status as a worker
An initial or an extension of a temporary resident permit

Appreciate any insight you can provide.
