Open work permit - IMM5710

Hi guys,

I’m applying for open work permit. And in IMM5710 I need to put my previous work history. I worked for a part time job for almost 1 year but I have left the company for 1-2 month due to family issues. Should I put 2 separate records there? If there’s no space how to add pages?
