OOPNP Nominee Application Form - Print Blank form help!!!

The Application form says to review the form in its entirety before printing, click the blue button" PRINT BLANK FORM. On doing same the form comes up blank despite my completing the application prior. It also has at the top a box

Office Use Only
Date Received Time Received
This form is to be completed

Please guide me on this one. If I print the completed application I do not see this box. Help!!! What am I doing wrong?

U're clicking point blank form so it will print blank.
Their must be some other option too

What other options do you see ?

I can print the forms individually but the it comes up with the Print Blank Form radio at the top. It does not have the Office use which looks official. Anyone in a similar scenario?

Hey,

what you have to do is submit all information and then click next next till you go to the last page.

Then please click save so your pdf will be saved with your entered details.

Now click on review submitted info. Then it will open all the pages in that form. Now again go down and then click on Print Summary pages.

Now print it and sign it and also write N/A for fields which are blank.

Hope you get all the points what i mean.

Hi,

I tried same way but it shows me couple of places blank as mentioned below:

I tried to go to last page and then enter review summary page. so all the pages opened but I noticed couple of tick marks and city name was blank although I printed it. After that when I go down and click print form then those couple of tick marks and city name are blank. show what to do? Any other way to print?

Yes, there is a glitch when the form is printed. The city name disappears along with some checkmarks, but you can add that manually.
