If you do not have a surname on your passport, travel or identity document, enter all your given name(s) in the surname field, write “not applicable” or “N/A” in the given name field.

Actually as per CIC guideline , It should match with your passport document .
and in case you don't have family name (surname) in passport use first name in family name section.

Nope, this rule applies to only you and Your spouse name. only for these fields you have separates blocks in generic form (as i remember correctly ) , for family form write complete name for your family members i.e. ABCD XYZ
