online OWP

So I am in the process of applying online for my extension of work permit and I answered all the questions. But the thing is that the required docs are: passport, proof of medical exam, stat declaration of common law union and the photo. proof of med exam and stat declaration doesn't make sense to me because im only applying for extension of my OWP and how do I get this again. I have the declaration form which I have last January and im not sure if that is still valid.

can somebody shed on me on this please or anybody who is in the same boat?

maybe you are right but the reason why I posted here is because I am under family class sponsorship..my commonlaw partner sponsored me. and I have done my medical, sent my police clearance and sent in the stat declaration form but I wonder if that is still valid. cic said they are just waiting for my background check which is near to its 8 month period already.
