I'm sure ya'll are sick of this question being asked again and again... but there are so many exceptions :P
Tried searching for the answers, but it doesn't seem there are too many Canadian-American couples.

I am sponsoring ONLY my American spouse, with NO children or dependants. We are going outland route, but living in Canada.
When we booked the medical the DMP said bring 3 photos.

My check-list (IMM 5443E) says applicant needs 2 photos BUT
The country specific (USA) check-list says the applicant needs 4 photos.

I have 8 identical photos of my spouse, total.

Question
1) How many photos should be in the envelope attached to Schedule A

We're Canadian-American! :)

I didn't attach anything, I put the photos in a small envelope and labeled it. Your DMP is going to use one for their file, one for Ottawa and one to the paper to give back to you. You will need to send 4 with the application. So you need 7, but 8 is good too, you can keep the extra one as a memory haha :)

You need to include 4 photos with the application you are sending to CPC-M. The ones for the DMP are *in addition* to those and depend on your DMP. parker24's example is typical for MDs who do the lab work and x-rays on site (otherwise, they'll ask for extra to verify it's still the applicant there too)
