Notarizing an affidavit in US for different names

Hi all,

I am trying to get an affidavit notarized for the different names used in India and in the US. Does anyone have a format for the same? I found a few online, but not sure which one would be the best to use.

Also when I called and checked with the bank, they said that they can only notarize that I was there present in front of them presenting the documents. If I go to them with my bachelors degree certificate from India and the passport which has my name as it is in the US, would that be good enough?

I would really appreciate inputs from folks that are in the US.
