NOC4011 Uni Prof./lecturer: Reference Letter Format with standard duties

I am working as a Assistant Professor in a Management College under Punjab Technical University. Can some body please share format of the reference letter for NOC 4011 (Uni Professor / lecturer) with some standard duties, I will mold them as per my profile. Thanks for your help in advance.

some people suggested to use NOC 4021 as you are also teaching in a college and not in a university...
anyways i am still looking for the same answer..
