1. What would be considered my level of education and NOC code : I have completed my bachelors in homoeopathic medicine and surgery from medical school in India (4.5 year program plus 1 year of internship = 5.5 years)

2. I am paid for working more than 30 hours per week during my 12 years of work experience with a charitable trust and my designation at the trust is that of a medical officer.

3. will I be eligible for the canadian express entry FSW visa?

You need to look at your actual job duties. Are they that of a doctor? The nature of your practical experience is what matters most in assessing your NOC. You need to convince the immigration officers that what you did as a medical officer matches the description of the NOC you select for your application. I hope it makes sense.
