No. of photos for Pr application snt to Buffalo

Hi,
I just sent in my husband's application for PR. In the document checklist it says for Buffalo applicants the number of photos to be attached in 4. However, is this in addition to the one in the form where they have left a blank space for applicant's photo or do I need to sent in only 4 photos all together?w Can anyone please advise...

Just the photos... dont attach any pictures to the form.
