no name or address in CIC receipt

Hi,

I am a CEC June 2012 appicant. In order to add my spouse to the application, I have paid the fees for him thru cic online. However, I don't see my name or address in the receipt.
Is it like that always or any issues with my receipt? I need to send this receipt along with my application.
Please advise.

Also, should I need to send the fee payment form(IMM5620E) as well ? I dont see an option for online payment in it. Kindly help!!!

You print the receipt and you fill it manually
