No information sheet emailed from the doctor and they said they submit?!

Should there be an information sheet for medical exam? otherwise what we could submit to CIC?

The dr if they are on eMedical should give you an information sheet with your photo on it.

Yes, they are eMedical, that is what I am confused, my wife have that exam in Minnesota, I did that in Texas, I have information sheet, but her Dr. didn't give her..but just called her and told her she passed...

She still needs to upload evidence of her medical test.

If the dr didn't keep a copy of her eMedical sheet he may have to hand write a letter with her UCI number.

Also, he doesn't pass her. Cic do.
