Is it true that

"Since March 2012, Citizenship and Immigration Canada requires that children who are less than 17 years of age and studying in Canada without a parent must complete and include the Custodianship Declaration (IMM5646) with their study permit application. It is mandatory that this notarized document is submitted every time an international student (less than 17 years of age) applies for the extension of a study permit"

Questions:

If so, can we use the old signed IMM5646 form(that was used in the initial study permit application) for renewal? Or we need to re-do a signed IMM5646 form again?
 
I think it should be recently signed. That's how legal thing works and custodian declaration is a legal document so it should be recently signed by guardian.
