Hi ,
I am in process of applying PR for my Wife. We are cousins from my Mother side. A quick question for you all. Do i need to mention that we are related ?.
If yes, then what do we mention in the form about our first meeting cz we were constantly in touch since childhood.

You need to be 100% honest with everything, so of course you have to mention you are related, because I beleive in one of the forms, it does ask you if you are related to your wife. Lying on the application can be grounds for misrepresentation.
 
Yes, you have to mention it. There is a question on the forms about whether you two were related before marriage. For the 'first meeting' question, explain that you have known each other since childhood (be as specific about the year as possible). When you are explaining how your relationship developed, you can explain how and when you two decided marriage was right for you.

It is legal to marry your cousin in Canada (though it is not common), and the visa officers are aware that it is common in certain cultures, so it should be fine.
