Need help with form imm5409e.. Common-law declaration- Student Visa

Anyone who filled up imm 5409e, please share your opinion on who needs to administer the declaration? Who is the right person to administer this declaration and sign the form and where should I get it done in India?

Any advise is really appreciated..

Yes, I figured it out on my own. We filled up the form and got it signed by notary public in my home town. They charged me 200 rupees and this took no more than 10 minutes.
so you can also get it signed/stamped by visiting your local notary public.

Just so you know, our application was refused as the officer was not satisfied that we will leave Canada at the end of our authorised stay.
