Need help to fill schedule A

hi there
i received my 2nd update request from Warsaw office
they asked me to fill schedule A for me & my wife
the question is:
the last section of the form under the heading DO NOT COMPLETE THE FOLLOWING SECTION NOW!! contains a signature, should sign this section or leave all empty & send the form signed only & filled the 1st section??

You should sign and put date on page 4 of 4 in the signature box under the declaration of applicant, which is just above the " DO NOT COMPLETE THE FOLLOWING SECTION NOW"
