Need Help... Query

I received ITA for express entry and was planning to get upfront medical for myself and spouse .The designated panel in philadelphia , US says that they will only do medical for applicant who has name on express entry form.
They will only do for spouse when ITA says so.
The spouse medical is required form on ITA. how should i proceed. please help?

Explain the fact that you won't her his/her name in the ITA letter, if they refuse to do the medical you'll have to try with another clinic.
