Hello friends!

I need some help here. My husband's medicals have now expired and he is going to redo them. We do not want any further delays in the application as we have been waiting for about a year now. I just need some clarification/confirmation on a few things:

1) Is Appendix C the appendix that my husband needs to fill out and give to the DPM? Because last time it was Appendix D. I just want confirmation that it is Appendix C from this document http://www.cic.gc.ca/english/pdf/kits/guides/3901e.pdf that my husband needs?

2) My husband is redoing his medicals with the same DPM that he went to last time. When he goes to this Doctor, is there anything specific that he should tell the doctor, such as, should he tell the doctor he's here to redo his medicals?

3) The first time that my husband done his medicals he had to submit the original copy 2 of the IMM 1017? Does he have to submit this form again? If so, where does he have to send it? If he doesn't have to submit the form, then will the doctor give him this form to keep or will the doctor keep form IMM 1017?

We just don't want to make any mistakes, especially when it comes to submitting any documents because, like I said, we don't want any delays. So if someone can outline exactly what needs to be done so that no mistakes are made I would greatly appreciate it.

Thank you so much.

Re: Need Help on Forms

Has the visa office asked your husband to redo the medical? If not, then it's better that he waits for it. I believe they usually send the forms that are needed along with the request to redo them.
If he has been asked and didn't get the forms... well, I could only guess what he needs to do, so I'll leave it to someone else to answer it.

No, my husband did not receive a request letter from the visa office to redo his medicals. We called up the officer that is dealing with my husband's case (the man that conducted the interview) and asked him what are we supposed to do if the medicals go expired. He told me that we can wait until the background checks clear and a letter is sent out to your husband to redo the medicals or he can go and redo them now. I told him that we don't want the process to be delayed any further than it already has been and I asked him what should we do, he says it's up to you, but if you want to save time he says that my husband can redo the medicals now as it will take a month or two for them to clear. By the time the new medicals clear we're hoping that the background checks will clear as well. This man told us that we do not have to submit anything, just fill out the appendix, give it to the DMP and redo the medicals. But I just wanted to confirm with others that had to redo their medicals if that is all they had to do or is there more to it.

The appropriate form and instructions are in your Region Specific Guide. My husband is re-doing his medical too WITHOUT a letter from the embassy. We are using the same form as last time and we wrote his immigration file number at the bottom of the form to let the DMP know that this is a re-do. Don't forget the pictures they request. Your husband will be responsible for submitting to the embassy the form given to him by the DMP when the medical is complete.
