Need Help in Form filling

HI FRIENDS,

I am studying in canada and for my co op work permit i need to fill this form
but i am unable to validate it.... my full name is Amardeep Singh (as on my passport... but there
are two columns under full name....... family name and given name... in given name i filled my name
Amardeep Singh.... what about the family name... without it i'm unable to validate this form... please
anyone help me out to fix it........thanks in advance...a.d

thanks for ur response buddy...there are only two columns ... family name ...and given name.... just tell me
what to put in family name... and what in given name... so that there will be no problem in future

Surname=family name (typically, other people in your family may use this name)
Given name=the name that is given specifically to you by your parents (I guess that's Amardeep)

Hy enfield ..m in a same situation ... My passport shows given name Jaspreet Kaur and no family name ... It is blank on the passprt ... Hw do i put it in the form tht has two columns as mentioned here ... ie. Family name and Given name .. wud it b Kaur and Jaspreet in my case ... ?,

But thts nt hw my passport shws it ...
