"In order for us to continue the review of your application, we require the following documents for you and your spouse:
Additional Family Information: A completed Additional Family Information form (IMM5645)"
Q1: Isn't this form typically asked for Visitor/Student purposes? If this is asked for my PR application, any insights into what it typically means?
Q2: The form is required for my spouse as well. So, when she fills out, will the "Applicant" still be me and she be the "Spouse"?
Q3: Do I have to fill in the names in both English AND my native language?
If anyone can help me out with these queries, I would really really appreciate it!
