Need help did not receive medical instruction doc for one member

I applied for Saskatchewan nomination program. I am the principal applicant. My family received the medical instruction documents. However mine is missing. COuld you please let me know who to contact to resend my medical instruction document

Also the name format is incorrect on the med inst document for my daughter. can i advise the doctor to correct it
