Just go to your local police station - identification department, tell them that you needed to get your fingerprints, asked for the calling card of the officer who assisted you for your finger prints. After getting your fingerprints have that form notarized by the duky authorized representative of the Philippine Consulate in your Province. After being notarized send it to Philippine Consulate Vancouver for consularization. Once it is returned to you, you can send it to Philippines and have one of your family member apply it on your behalf.
 
applied for my NBI in October 2014 until now wala pa rin...I mailed it directly to Ms. Julie Macalit but no NBI clearance sent back to me I was told later that its not advisable to mail it directly kasi hinde aasikasuhin and probably its lost now...now I ask my cousin if they can find my application form if not then I have to do all over again..we drove 5 hours to Vancouver to get the form and completed it...its just so frustrating with the phil govt...have someone to process it in your behalf makukuha ata within two weeks or less...
