Name mismatch in Salary Slip and Passport

Hello,

I have received an invitation for the express entry under CEC. I show my Indian experience and to prove that I need to provide salary slips for 1 year. In my salary slips my name is "Vivek Patel"
while on my passport name is "Vivekkumar Patel". I found that for this one I need to do notary but my question is that what kind of notary that would be?

Would this notary be stating that both person are same or Those are original documents. Please reply I need urgent help on this.
