I need some expert advice preferably from seniors on the below query.

My full name goes as Xxxxxxx Yyyyyabcd Zzzzzzzzz as in birth certificate and national ID card. The 'abcd' part is like a suffix to the middle name and is joined with it.

However in my passport and in some other docs such as my child's birth certificate, etc the name appears as
Xxxxxxx Yyyyy Zzzzzzzzz (without the abcd part).

In all other documents such as work contracts, experience letters, WES, degree certificates my name appears as
Xxxxxxx Y. Zzzzzzzzz (the middle name is denoted by an initial).


My questions,

a) Wherever my name appears as Xxxxxxx Yyyyyabcd Zzzzzzzzz and Xxxxxxx Yyyyy Zzzzzzzzz (without the abcd part) I will require an affidavit to certify that both names are of the same person. In this affidavit do I have to add a list of all of the documents that have this name discrepancy?

b) Do I need to include the name Xxxxxxx Y. Zzzzzzzzz (the middle name is denoted by an initial) in the affidavit too certifying that this name also refers to the same person? and add a list of those documents too?

even though lawyer or notary people have standard formats for such cases & would guide you, still here how it goes-

better to mention all documents with which names is mentioned on which all docs,

& statement that all above names belong to the same person...


rest things person making the affidavit will take care, this is a common affidavit... don't worry!!

Even though Initials should work just fine, when you are at it, why not?

Just adding another few document names in the affidavit will cover all the documents and all name versions so better to include!!
