Hello, I need to separate my family name into given name and last name due to issues of my given name being blank. I am applying for a passport for the first time. Can I just write it separated on the application form or do I need supporting documents? I just need to split my name, I am not adding any new names. Do we have to get the name change certificate from the province for just reversing/splitting names too?
