I have signed and dated my spousal sponsorship application on November 1. 12 days later I mailed it. Is it ok to have such gap between date on application and mailing date?.

It doesn't matter about your signature date but you have to be careful if the forms had been updated on CIC website after you downloaded.
 
They update one form IMM0008 after I sent my application package. Is this ok, or they will request to send the updated one?

Its fine nothing to worry about... I had signatures from April..May...June...and July and application was sent and received in July.

It's fine if the form was updated after you sent yours.

It happened to me the same that I found one form was updated right after I sent out my application.
