Hi,

I went to Medical today and doctor informed me that my Passport information (primary applicant) is incorrect in their system.

Instead of my passport information my daughter passport information was added.

Do I need to inform CEC about this.

Yes, you should inform CIC of the error as soon as possible. What do you see on the form that was given to you?
 
On the form - it has my correct name, address, but medical form doesn't have Passport information.

But doctor showed me in the system that has incorrect Passport information.

Also - to whom I have to email this?

Email CPC Ottawa. They will be able to rectify it. It will also be a good idea to order GCMS notes

Yes. You can. But you will need to pay the CAD 5.00. If I am not mistaken, you will be applying under "Privacy Act".

Just go with the flow on the form you will figure it out.

That does create a problem as ATIP explicitly requires that you are in Canada. I can't comment on this as I was always in Canada, but others might be able to better answer.

If you are physically in Canada, you can obtain them for free under the Privacy Act.
If you are overseas, someone else (a Citizen, Permanent Resident, or someone physically present in Canada) will have to obtain them for you under the Access to Information Act. This can be a relative, friend or sponsor, or a commercial service. For this there is a $5 fee (more if you use a commercial service), and you will have to provide consent.

Average time you will get your GCMS notes: 30 days

^That's a really detailed and helpful post. I guess I got ripped off for $5 as I'm in Canada and didn't have to pay for it :)

Yeah I was going through that webpage and made the wrong selection somewhere in the dropdown lists. I was like "5 dollars? Whatever just take my money" :P
