Missed first deadline for the first questionnaire,

Although I filled up and sent back the questionnaire,surprisingly today I received a letter that I had missed the first deadline of my initial questionnaire which was in September 2013!!!, Now I have received another notice (final notice) asking to send the questionnaire again in 30 days. The letter has been sent from Scarborough, Ontario. I am wondering about one thing, may someone knows about this case please help me:

1. Should I do the questionnaire over with new recent date which I am filling it up, or should the dates and supplementary documents be dated the same as my application was submitted.(so it was submitted about 2 years ago.)

and also they didn't send me any new questionnaire!!!!

suggest you pick up the phone and contact cic and get the clearance,

first RQ send by registered post ? you must have prove.

better rush otherwise they will suspend the file...
