Hi! Maybe someone received this form for SOWP in GC key before. I am SP holder and I got this for my spouse along with Authorization To Travel. I read that they are giving it to PR’s but we are not them and are coming only for 2 years.
A little of history - my husband had issues with X-ray during his medical exam and had to do a follow up for 3 months in suspicion of tuberculosis. The last appointment showed it as permanent damage due to his soldiering job and he was let go
