I am an Indian resident living in UK, I am sure if I had applied visa from India, I do need a medical but do I need medical if I am applying from UK. If I need, shall I just write an application to visa officer for a medical test.

Yes you MUST take a med test.
No. Follow www.cic.gc.ca request.
Find a doctor who works with CIC and just make an appointment. Doctor will know what to do.
