Medical Request Form Query

Hi,

My wife recently got notified she has been approved as a sponsor and I have received an email for a medical request as I didn't do one upfront. The email states:

"An original Upfront Medical Report form (IMM1017B UPFRONT) signed by a Panel Physician or an E-Medical Up-front Medical Notification information sheet has not been provided. You must complete the medical and once completed forward this information directly to our office noted below. "

When I called up to book my medical the worker said I would need a form that was attached to my medical request email from the CIC, that would be partly filled out by them (IMM 1017E?), however I didn't receive any form with the email. Does this mean that I should go ahead with the medical as if I am doing it upfront and fill out the IMM1017B form as stated in the email? Or has the CIC forgot to attach the form I need, meaning I have to contact them and ask for it?

Inform the Dr that you are doing an upfront medical You do not need any forms. This will all be done electronically and your Dr will send the results cic . Once this has been completed send the receipt off to the office of the address they provided in your email
