Hi,
I had my medicals done two days back and just got the reports in my email today. Upon checking, I see that the doctor has said "No" to the option "Are you taking any prescribed pills or medication". I had told the doctor that I take prescription medication when he had asked me about it, so I am assuming it must have been a human error on his side.
Question 1: Do I call the doctor to fix this issue? Or do I just let it go? I am just worried about misrepresentation as I am willingly suppressing facts if I let it go.
Question 2: I have received 3 forms from the doctor, which form(s) to upload? Name of Form 1: Information sheet. In this form apart from personal details, there are other details like IME: non EDE and Upfront Med type: Worker Name of Form 2: Tracking sheet Name of Form 3: Health Case details. This form has details about all tests conducted and how I fared in each of these tests. This is the form where the doctor has said "No" to prescription medication

Strange enough, no medical panel team shares report with the applicant. They are not supposed to, unless you have specially requested and paid for it separately.
No idea what one would do in this situation except for checking with the med team itself

Do you know which form to upload? Form 1, 2, 3 or all? Based on your answer looks like Form 3 should not be uploaded.
 
If this is your upfront medical, you will need to upload the medical information sheet you must have received from the med team

I went for upfront medicals. Not sure what to do with the error on the doctor's side.

Can one of you please comment on if I can overlook the error or doctor's side.
 
^ If you mentioned to the doctor what your prescription medication was, it probably was something that wasn't really that they're looking for for medical inadmissability.
