Medical Report Query

Hello everyone,

So I received an invitation to apply for the express entry PR. I was going through the documents required for the medical examination and saw that I am required to carry IMM 1017E for my medical exam. However, I haven't received any form. Please suggest me what should I be doing? There's no email id or thread on Cic which caters to this problem.

you need to carry your ITA and original passport to the medical exam center. You may also want to check with center for any other documents. Best luck!!
