Hi everyone,

I am new to this forum and hope someone could please help on this matter.

I am going to take medical exam, may I ask if that would be for the whole family under my application? Also at my place, they said they had 2 forms IMM 1017 E and IMM 1017EFC, which one is the correct one to you?

I do not have an idea about your 2nd question but your spouse and dependent children need to take the medical tests, if you plan to include them in your application and that they'd land along with you.
 
You don't need any form as you're doing upfront medical exam. Find a panel physician in your country and Book your exam and tell them you need non-ede/worker upfront exam for the whole family.
(even they may not go to Canada with you.)

thank you very much spth38, it has cleared things up for me a bit. So basically under express entry we have both options of waiting for instruction and get the medical check upfront?
 
Non-upfront is probably for working visa or family visa. Every express entry participant here I know is doing upfront.

There is no rush if you don't have an ITA yet because your upfront exam only last one year. (You must land before it expires or take the exam again)

Once you receive ITA, you will answer a bunch of questions and then see a section where CIC ask for your medical exam. When you go for the medical exam and you will receive a Information Sheet with your UMI (a medical ID which can be used by CIC to track your record). You should scan the information sheet and upload it to that section.
