Medical Receipts along with Passports

I wanted to know if we have to send oroginal Medical Receipts along with the Passports.

I have sent the originals with Passport on 21-april-2010.Do keep photocopys of the same.

Also I woudl be attaching Bank Certifcate. Not attaching any Bank Statement. Is this fine???
