When having a medical done, for a US to Canada immigration - my husband was told by the secretary at the doctor that we get nothing back - it gets sent directly to Ottawa. Is this true? I thought we had to submit something with our package?

You get a green sheet back. I think the receptionist must be very confused. Be SURE to get this sheet, it needs to go with your application so they can match it up.
 
You should get a copy of IMM 1017 EFE to submit with your application.

This protected form has a unique serial number on it. The VO uses that serial number to match up your medicals with your application.
 
Out-land applications should have an appendix C sent with the application... you should have read and printed it out and taken it with you, are you 100% sure you told them it was an out-land case? call and double check cus In-land and Out-land medicals go to different places... below is a link that will help you... should have been in your Out-land application kit...GL!

Edited to add... the form Computergeek is speaking about is for In-land applications... I was assuming you were talkin about Out-land since you said Ottawa... Out-land goes to Ottawa In-land goes to Vegreville... but yes either way you should have had something.
 
Sorry, it's IMM 1017 EFC (exempt family class) and I definitely still have a scan of my copy from the DMP and it was submitted with the application to Mississauga. IMM 1017 DCF is also acceptable (the French version of the same form).

IMM 3910 is specific to the US; each region has its own instructions and checklist. Appendix 'C' is the form I took TO the DMP. IMM 1017 EFC is the form they gave me back (the "receipt" for the medicals.) The bottom of the form says "Copy 1 - Regional Medical Office, Copy 2 - Person Examined".

As far as I know, inland uses the same form.
 
We haven't done the medical yet, he had called to get the info - and that's the info he was given, and that's why we're confused.

I guess it's different going to an In-land doctor for and Out-land application, I went to a doctor in the states, printed my appendix C when I got there receptionist told me they had new forms from CIC and she pulled out the Appendix C in triplicate stapled my pictures to it, stuck one in my exam folder, one in the x-ray folder and gave me my copy. But if you havn't done it yet double check, heck triple check to make sure they are doing it Out-land I would be more worried about it going to the wrong VO then which form they gave me as a receipt.

Well this may also be something new (I did my last medical in February 2012 and had the results back five weeks later.) CIC has been moving away from protected forms due to expense and towards plain paper.

That's not the form I have - mine is IMM 1017 EFC E. I remember the receptionist wasn't familiar with the Appendix 'C' and had to ask someone else in the office about it and they dug out the form.
