Do the 5 photos that someone takes to the medical have to
be the same size as the photo specifications in appendix B
or can they be regular passport photo size. Please help,
my medical is tomorrow and I want to be sure.

I can't be a 100% certain by the medical office here in North Carolina said the regular 2 by 2's taken for passport photo's are fine for medicals

I took the same size as the ones listed in the appendix. They told me passport sized but I brought ones sized to the appendix specs and they didn't say anything. I'm pretty sure if you want to play it safe take the size listed in the appendix.
 
I printed the same pictures as for in the application extra for the medicals too. That way, the pictures are fully identical, so they can confirm the identity (that's what it's all about, I suppose).

FWIW Canadian passport size pictures are much bigger than the immigration sized ones.
 
UK passport photos are the same size as Immigration photos, so I suppose it depends on where you are.
 
There is no way a passport sized photo will be acceptable when you are issued the Visa. The small sized immigration photo is what is used for the photo on your PR card.
