Do I need to bring anything with me when I go do my medicals in a few weeks? The receptionist seemed to think I did. I do know that I need to bring my passport and photos. But I want to make sure before hand.

The following documents you will require for doing your medical exam.

1. Original Current valid Passport.
2. Two set of photocopies of your Passport's first two pages and last two pages.
3. 8 Photographs according to the CIC's instructions.

That's what I thought but she ( the receptionist ) said to bring my forms. But I thought they didn't use forms anymore so I I wanted to check.

It depends on whether your doctor follows the old or the new procedure. Under the old procedure you had to take with you your passport as proof of identity, 8 photos and the yellow form which as I recollect was IMM 1017 (from memory). However under the new procedure everything except the passport is for practical purposes redundant: when I went to my medical in Canada last April my yellow form and my photos were returned to me unused. Instead the doctor's receptionist took my photograph with an electronic camera, informing me that it was easier to incorporate it in the data she would in due course forward electronically to Ottawa, and after the examination provided me with a printout of the first page of the medical report, not with the completed yellow form. That does not mean that you should not take with you the yellow form and the 8 photos; but you may find that they will turn out to have been unnecessary.

Makes sense. I will bring those. Just in case. If they use them great, if not Meh. I don't think I will call to ask though. The lady probably won't know any more the second go around.
 
Having my husbands done next month and they told us , just his passport and a cheque or cash . They will take the photos and summit when CIC request them ... and the best part $190.00 total . It was going to cost 300.00 pounds in the U.k.
 
Cool. I couldn't even find the 1017E form on the site. I had an old 1017EDE form I got for proof gathering /can't helpful purposes but that was a while ago and it has changed since then as the revision for that o one was 2004.
