My medical forms arrived before after my marriage, so the marital status there was single instead of married.
I added my wife, today we received my spouse's medical form, but instead of my last name, her maiden name is the one that appears on the form.
my wife already use my last name,
Was that how CIC does it, or should it be my last name ?
Pls. guys help me, if anyone from you already had similar situation like this....
