Medical form

I a doctor for my appointment here in Kingston. I had to go and pay 50 to hold the appt. Did that yesterday.
I know I have to take the photos and appendix c and the doctor will have Section A( I asked they had to dig for it but found one)
BUT in the paperwork thet gave me...it states they WILL NOT give me any paperwork back that they will send in.
I know I need to send it it in...I called back and the sec said no they send it in.....but I tried to say...I have NO file or number yet...will not till I send it in.

1)So...Do I have this wrong?
2)Is there something I can print to take with me to prove YES I need to take it with me to send in.

You will be given a copy of the medical form. That is what you are to submit with your application as proof that you have undergone the exam. You will not receive the results of the tests.
