I am sponsoring my husband from the Dominican Republic to come to Canada. I recently received the medical form for him and his children to complete their medical assessments. The date of birth is incorrect for two of the children and the ime number/barcode is not on the form. How do I go about requesting the revised forms? And how do I get a form with the ime barcode?
